Application Form
Completing the Application
All lines MUST be completed on the application!!
Please use the boxes under First Choice and Second Choice to indicate your choice of class (do not simply circle one of the Days Offered).
If something does not apply to you, write N/A on the line. Pay particular attention to work addresses including zip codes and work phone numbers. If you, or your spouse, operate a business out of your home, please include the company name and address as you would on your tax form. Also, if you have a nanny or permanent sitter, provide their complete address and phone number.
NOTE: Incomplete forms will not be accepted and your child will not be enrolled until a complete application and agreement form has been accepted!
Forms due at the time of application are:
1) a complete Application Form,
2) the completed Parent Agreement form and
3) your non-refundable payment for Registration.
Re: Medical Forms
SSUMC Preschool’s Medical Form and a current Immunization Form, both of which must be completed by your child’s pediatrician, and the Emergency Form are due by
Many physicians’ offices are charging for completion of forms that doesn’t occur at the annual well-checkup visit (that’s the one right around the child’s birthday). The SSUMC Preschool Medical Form is available on-line or in our office now. If your child’s annual well-checkup is scheduled for late winter or the spring, be sure to take this form with you!
